You will need to download and complete an application form and a payment form. Please click on the appropriate form to download it.

Important!
To complete your application process we need you to send us a completed Application Form, a Payment Request Form and the Customer Declaration Form (which you will receive directly from NBN Co). If you have not registered your details with NBN Co, you will need to do so my calling 1800 881 816 or registering online.
What happens Next?
Once we received the above paperwork from you, we will complete your application process and call you to confirm that everything is good to go. We will then notify our installation technicians who will contact you shortly thereafter to arrange a time and date for installation. Installation is usually completed within 2 - 4 weeks after we receive your completed forms.
